1. De-identification shall be done at the PHCP level for immediate statistical reference.
2. The PHCP will transmit information from patient's records to PHIE as shared health record. If the patient consents, the patient's health record may be processed in PHIE without the need of de-identification. If the patient does not consent to participate in PHIE, the patient's health information shall be de-identified containing only information necessary for generation of health statistics.
3. Only de-identified health information shall be stored in the PHIE Data Warehouse.
