Collection of information shall start at the time of registration in the health facility. This shall be done in the Admitting/Registration section and subsequent information shall be provided at different points of care undergone by the patient.

The Privacy Officer (or a duly authorized representative) shall be responsible for the orientation of the patients regarding PHIE implementation and validation of patient's information.

The time of encoding and processing of patient's information shall be specified. In facilities with less available computing resources, it is suggested that encoding should be done before patient discharge.
(If hospital does not have EMR in place, encoding and processing of patient information will be through the medical records section or hospital information management section. If they have an EMR, or Hospital Information System, encoding and processing begins at the service transactions covered by the HIS.)
