Family information, socio-economic profile and other vital information shall be collected by the Medical Social Worker or some equivalent personnel in the health facility.
Data collection and processing shall be done by a permanent employee of the health facility.
Additional information to be accessed (for Personally Identifiable Information) may include
* Previous illness, except highly communicable (HIV/AIDS)
* For highly communicable diseases, there should be special coding, and special documents to be signed by: patient, attending physician, and HAC team.
Special codes shall be given to patients with special health conditions (HIV, Ebola, MERS-COV).
GCP guidelines on changing data on information should be applied:
* Original entry should be visible
* Change must be dated and countersigned.
* Reasons for the changes must be entered or specified.
In compliance with Act No. 3573 otherwise known as “Law on Reporting of Communicable Diseases”, all notifiable disease/syndrome/events and conditions shall be immediately collected and reported to the local and national health authorities.
